This form must be completed by all athletes taking part in TOHS Track & Field and their parent/guardian. The form gathers basic information about the athlete and includes an acknowledgement of the syllabus and a donation commitment. The form is completed and submitted online.

The athletic clearance process MUST be completed BEFORE s student athlete will be able to practice or compete.  Student athletes must obtain a physician's clearance to participate in athletic activities each year.

If your athlete is NOT taking the bus, you MUST complete/submit the transportation permission form.  This form must be completed each time a parent wants to drive their child, or have their child drive, to or from games, events, etc. and submitted to Coach Marlene.

This form is to allow your athlete to attend 7th period Athletics PE on a “TO BE ARRANGED” basis.  The hours will depend upon practice time, game day schedule, bus departure time and/or facility availability.
