The athletic clearance process MUST be completed BEFORE your athlete will be able to practice or compete.
Step 1: Get a Physical or Sports Clearance from any doctor. Use the form located on tohsathletics.org website.
Step 2: Create an online account atathleticclearance.com and click on the California icon. Click on the “Register” button.
Physical and Athletic Clearances are valid for 12 months from date of the exam. The Athletic Clearance process is managed by the TOHS Athletic Department. See below for more detailed information.

Go to athleticclearance.com to begin creating your account. You will need to create an account in order to begin. Once you have done that, please click on the button that says Start Clearance Here! You will then select the year, school, and sport for your athlete. Please be sure you select the correct school year. You will need to do a separate clearance for each student athlete in your house. Only one parent account is needed regardless of how many athletes you have at TOHS. There is a tutorial video on the home page if you need further assistance.
All forms require signatures which can be done as electronic signature of both athlete and parent. The doctor’s physical form does need to be printed from the site, filled out in its entirety, signed and dated by the doctor, and then uploaded back into the site.

Please check to ensure the doctor has written the date of the physical on the form. We also require a copy of the students’ medical insurance card to be uploaded for clearance. This will store all uploads into the document library.
**To migrate uploads from the document library to the individual files on the website:
If you upload forms to the “documents library” section of athletic clearance, you will still have to select the file from that location to upload it to your child’s profile. Saving it to the documents library does not make it visible to the athletic department. To do so, click on the pencil next to the word “Physical” on your child’s home page. This will allow you to upload the files to your child’s profile.
Once all forms have been completed online, you will receive a confirmation that your student athlete is clear for their season. This clearance is generated by the athletic department once we have reviewed all forms as being complete.

The physical exam is valid for 12 months from date of exam.
PLEASE NOTE: Any student who has transferred to TOHS in the 12 months prior to a season must contact the Athletic Director.
